COVID Testing Confirmed – Thursday January 21

Our Diocesan COVID testing has been confirmed for Thursday January 21 at 11:00 am. The consent form for the slip is below:

SOMOS Student Parental Consent Form

For those of you who completed the original form in November, the only updates to the consent form is a change from the name of the testing provider (MyOnSite to SOMOS) and type of test (Rapid Test to PCR Test). If you have already sent in the consent form and accept these changes, please complete the following link, or you could send a simple email or St.AdalbertRemind stating you consent to the two changes or / complete another form (and email/send in to school), if you prefer.

If your child had been tested two weeks from Thursday, please make sure the school has your student’s result which will count towards our 20% tested. If you have any questions please let us know.
